The above-named Educational Unit and each Educational Unit for which this Assurance
is given hereby agrees that it will not discriminate on the basis of sex in the admis-
sion of individuals to any training program of the Educational Unit, as required by
Section 799A of Title VII of the Public Health Service Act, Discrimination on the
Basis of Sex Prohibited, and Section 845 of Title VIII of the Public Health Service
Act, Prohibition Against Discrimination by Schools on the Basis of Sex, as applicable.
This Assurance obligates each Educational Unit from the time any award under Titles
VII or VIII is made to the Educational Unit. Each Educational Unit also agrees that
it will comply with all Regulations issued by the Department of Health, Education, and
Welfare pursuant to Sections 215(b) , 799A, or 845 of the Act. Each Educational Unit
has received a copy of, and understands the contents of, the "Explanation of Assurance"
describing the obligations of the Educational Unit under Sections 799A and 845.
Each Educational Unit further agrees that it will not operate any training program
funded under Title VII or Title VIII of the Public Health Service Act for the benefit
of, or in affiliation with, any other Educational Unit which discriminates on the basis
of sex in the admission of individuals to any of the training programs of that Educa-
tional Unit. Each Educational Unit further agrees that it will operate training pro-
grams for the benefit of, or in affiliation with, any other Educational Unit only upon
ascertaining that such other Educational Unit has submitted an Assurance of Compliance
to the Office for Civil Rights, HEW, and that the Assurance has not been rejected.
Each Educational Unit further agrees to furnish such information, and to permit such
reasonable access to its records and other information relating to its obligations
under Sections 799A and 845, as may from time to time be requested by the Director,
Office for Civil Rights, HEW.

*Signers of this Assurance are cautioned that terms herein are defined, for purposes
of this Assurance and of compliance with Sections 799A and 845, in the "Explanation
of Assurance, " HEW-590A (3/72). If a college or university includes more than one
Educational Unit at the same location the College or University may, if it chooses,
file a single Assurance for all such Educational Units which will apply to all train-
ing programs of those Units. If this is done, attach a list of all Educational Units
to which this Assurance applies.
